Clinical Study of Generalized Peritonitis due to Heal Perforation


Abstract
Over a period of 16 years, from January 1960 to December 1975, 260 patients with genera-lized peritonitis due to ileal perforation were observed at National Medical center.
Of 260 patients with acute diffuse peritonitis due to perforation of terminal ileum, 116 cases (44.6%) were due to trauma, 96 cases (36.9%) due to salmonellosis, 24 cases (9.2%) due to tuberculosis and 4 cases (1.5%) due to intestinal obstruction, 2 cases (0.8%) due to Crohn¢¥s disease, -2 cases (0.8%) due to foreign body, 2 cases (0.8%) due to cancer, 2 cases (0.8%) due to incacerated hernia, 12 cases (4.6%) due to other. causes. Among these cases, clinical evaluation was made for this 96 cases.
1. The sex distribution assumed a ratio of 1.7 to 1 with-61 males and 35 females.
The age in the peak incidence; was third decade and fourth decade of life (62. 5%).
2. The majority, of infection of salmonellosis occurred in summer and autumn (70%).
3. Perforation was developed mostly (59.4%) from 2nd week to 3rd week from the onset of
disease. Only 38.6% of perforated cases were operated within 24 hours after the onset of perforation.
4. Hematologic finding showed leukopenia in 12 cases (12.5%), normal range in 46. cases
(47.9%), leucocytosis in 38 cases (39. 6%).
5. 70 cases (80.5%) revealed positive result in widal test.
6. Subdiaphragmatic free air shadow was detectedlin 62 cases(66.7%) with X-ray examination.
7. In the preoperative clinical diagnosis, 59 cases (61.5%) were correct and others misdiag-nosed.
8. The most predilection site of perforation was proximal 40cm to ileocecal junction(76.1%). Single perforation was in 87 cases (90. 6%).
9. The operative procedures were simple closure, 70 cases(72.9%), segmental resection and anastomosis, 24 cases (25%), and ileostomy, 2 cases. (2. 1%).
10. Wound infection and disruption was developed in 61.5% (49 cases) as a most cornmon complication.
11. Mortality rate after operation wag 12.5%o (12, cases), which included 9 cases of simple closures and 3 cases of segmental resection and anastomosis.
